A contraceptive vaccine underwent preliminary testing recently. The testing involved 30 female volunteers. No adverse effects were reported. The vaccine works by creating antibodies against HCG, a female hormone essential for conception. The volunteers all were infertile by virtue of prior tubal ligation.

After injection with the vaccine, they were studied to measure the levels of antibodies against HCG. The follow-up study indicated the vaccine produced effective antibody levels for six months after inoculation. Other trials will follow. There is no way of predicting just when such a vaccine will, if ever, be available to the public. The World Health Organization sponsored this testing.

Tissue Dear Dr Donohue: My ankles swell when I have to stand up for long periods. When I he down with my legs up, the swelling disappears. Are there medicines to cure this? F.K. I say this about so many ailments I must sound like a broken record: You have to first find the cause of a symptom before you can correct it.

Edema is the general term for such swelling. It means tissue is retaining fluid. That can be a local situation for example, blocked leg vein valves slowing circulation and allowing leakage of blood fluid into tissue. Or swelling can indicate a problem far from the scene for example, a weak heartbeat (heart failure) resulting in poor circulation. However, with heart-related edema, the fluid retention occurs throughout the body, not just in the ankles.

True, swollen ankles are usually an early sign, but it's only the visible tip of an iceberg. That's because of the added fac-. tors of distance from the heart and the force of gravity. I can't suggest a specific answer without knowing more. If leg veins are the problem, leg elevation, elastic stockings and avoidance of long periods of standing might work (see next item).

If it's heart failure, you may need drugs to strengthen the heart muscle. Finally, there are other causes of edema to be considered in diagnosis and treatment. You should see your doctor. Dear Dr. Donohue: I have a very bad case of varicose veins.

How dangerous are they, and what can be done for them? M.H. One in five adults has varicose veins, so they can't be too dangerous (or we'd be dropping like flies). For most they are nothing but a cosmetic problem. A few people may have an achy sensation or leg fluid accumulation (see above item). The advice for varicose veins lias not changed much over the years body rest, leg elevation, elastic support hose (hip length) and appropriate exercise.
