Ophthalmologist Some form of glaucoma is found in all age groups but is more common in those over age 35. Approximately 2 percent of those over age 35 have glaucoma. The tendency to develop the disease tends to run in families. Glaucoma has been called the thief in the night" because most glaucoma causes no pain or other symptoms. Glaucoma is a group of disorders in which the pressure within the eye is too high for the health of the eye.

Although for large populations, ranges of normal eye pressures have been determined, the pressure that is acceptable for an individual may be above or below these normal" values. In fact, about one-third of those with glaucoma, in the United States, have eye pressures that are within the range of normal for the overall population. Thus, the diagnosis for glaucoma cannot be based solely on a specific eye pressure reading. In order to properly diagnoseglaucoma, we have to know how it causes damage to the eye. In the back of the eye is a large casing of nerve fibers, the optic nerve, that carries visual information from the eye to the brain.

Glaucoma causes characteristic changes in the appearance of this nerve which can be detected when the eye is examined with an ophthalmoscope (an instrument used to view the back of the eye). As glaucoma causes injury to the information carrying fibers of the optic nerve, the damage is reflected in reduced peripheral vision that can be mapped by a visual field examination. The diagnosis of glaucoma depends on the presence of typical changes in the appearance of the optic nerve, coupled with abnormalities in the visual field. The presence of these findings implies glaucoma regardless of the eye pressure. Once the diagnosis of glaucoma has been made, we know that we must lower the patients eye pressure if further damage is to be prevented.

This can be accomplished with medications, laser surgery or surgery in the operating room. The American Academy of Ophthalmology suggests that all individuals, over age 35, be examined every two to three years for glaucoma. In summary, glaucoma is a treatable form of blindness that requires a careful eye examination, including an examination of the optic nerve, to make a proper diagnosis and to prevent needless blindness.
