it Oh By PHIL ROURA and TOM POSTER -v .7 -v. 4 j5 -4 RICHARD CORKERV DAILY NEWS Yoko albvsa causes stir John Lennon's widow, Yoko Ono, is scheduled to release her new album to radio stations this morning amid industry concern that the album cover may be too controversial for some fans. The album, entitled "Season of Glass," has a cover photograph that shows a pair of "blood-encrusted" glasses that "presumably" belonged to Lennon, a record industry spokesman said. Lennon was fatally shot outside his apartment here last Dec. 8.

Mayor Koch will honor Lennon today when he presents the Handel Medallion, the city's highest cultural award, to Yoko in a City Hall ceremony. VJho's afraid Carol? Carol Bellamy for governor? Against Hugh Carey? That's the impression that Washington-based pollster Peter Hart is leaving with some Democrats who have been subjects of telephone interviews by Hart researchers. Yesterday, City Council President Bellamy reiterated exactly what she's been saying for some time: If Carey runs for a third term, it's "highly unlikely" that she would run; but if Carey retires, it's a completely different ball game. Hart's office refused to discuss who they're taking a gubernatorial poll for. In the past, though, Hart has polled for Carey, and the odds are that he's doing his thing again, operating in behalf of the gov's media whiz, David Garth.

VJhaf, no gioves? When wealthy John W. Kluge walks into St Patrick's Cathedral tomorrow night, he'll be wearing white tie and tails for his wedding to Patricia Gay. In fact, the formal wedding attire for the male guests is white tie and tails and that has had some of the most important people in town scurrying around to rent the formal attire yesterday. Howard Samuels, for one, dashed back up to his favorite rental place yesterday on E. 86th St.

He'll be among the guests that include Lee Iacocca, George Steinbrenoer; Donald Trump, Sen. Al D'Amato and Bill Fugazy, who will be on hand to see Terence Cardinal Cooke and Bishop Theodore McCarrick officiate at the ceremony. After the wedding, the guests will attend a real old-fashioned wedding ball at the Metropolitan Club. Kluge, who isn't as tall as the stately bride-to be, recently converted to Catholicism to wed Gay. The media king, top man at Metromedia, owner of Channel 5, is going all out to make his wedding a big event It's something that will live in everyone's memory.

Maybe that's why it's being held on the Memorial holiday weekend. Pee Wee Reese will be back. Dsm Bums is making a camohazU. hero It's being billed as the largest reunion of former Brooklyn Dodgers since Dem Bums wui dragged to Los Angeles by Walter O'Malley. The conclave of old Dodgers will be held on June 2 at the Montauk Club, in Park Slope, in the shadows of old Ebbets Field, featuring the Dodger Sym-phoney orchestra and the sounds of yesteryear.

The bash is being pulled together by State Sen. Tom Bartoslewlci of old Greenpernt, who wants to bring the Dodgers back to Brooklyn not the old guys but a new professional baseball team. Gov. Carey will proclaim June 2 as Brooklyn Dodger Day in New York, hoping that this reunion will kick off a campaign (not his) to get the late Gil Hodges into the baseball Hall of Fame. Hodges' widow, Joan, who lives in Brooklyn, will attend along with Carl Ersklne, Clem Lablne, George Shuba, Gene Hermanski, Billy Herman, Joe Black, Pee Wee Reese and Ralph Branca, who tossed that memorable home run pitch that cost the Dodgers the pennant More of the old gang is expected at the grand slant affair as "The Dodgers Are Coming Home to Brooklyn." Sportscaster Marty Glickman will announce the players whom you may not recognize without a scorecard.

P.S. Of course, there's a bill involved in all this. It's in the Legislature and would create a temporary state commission to see if a major league baseball team could be brought back to Brooklyn. The bill is sponsored by Bartosiewicz and Assemblyman John Branca, who just happens to be Ralph's kid brother. Lamarr displays her new look last night.

Medy: gflve esse nay. oyBw Hedy" Lamarr came out of dark seclusion last night to see her close friend and ophthalmologist, Dr. Charles Kelman, at the Friars Club, but all eyes were on the retired screen beauty whose face and eyes thrilled movie fans around the world for more than three decades. Hedy has been a recluse in her Manhattan apartment because of her failing eyesight "I was blind for more than seven years," she told our Richard Corkery. "But I'm fine now.

Dr. Kelman gave me my sight back. He gave me my eyes." Kelman operated on Hedy at Lydia E. Hall Hospital in Freeport, L.I., last October. He removed cataracts on both eyes and implanted new lenses.

Until last night, Hedy had avoided the spotlight But she looked ravishing and far younger than her 65 years. "She feels great and would like to do some modeling work, TV commercials and whatever else is available for her," said a friend. Hedy joined the Friars Club audience on celebrity night to watch Kelman, who performs as a standup comedian and singer. After the show, she led the cheers for her doctor.
