Ted was born on September 24, 1938 in Uniontown, Pennsylvania to the late Michael and Nellie (Matthews) Churby.

He was preceded in death by his sisters, Beulah Churby and Eva Kezmarski and brothers, Jack, Harry (Ruth) and Joseph (Sylvia) Churby. He is survived by his wife of 52 years, Linda; two sons, Scott A. (Sandra) Churby of West Salem, Ohio and Bradley C. Churby of Springboro, Ohio; six grandchildren; a sister, Dolly (Charles) Zerby; and two brothers, Thomas (Nancy) Churby and Edgar (Jeannie) Churby. Ted served in the U.S.

Army as a tank driver and retired from Standard Products in Cleveland, Ohio and also Plastic Trim in Beavercreek, Ohio as a Plastics Engineer. Mary was born to the late Raymond Myers and Helen (Putnam) Myers, February 10, 1925 in Cincinnati, Ohio. She was a 1943 graduate of Norwood High School. Mary was a longtime member of the Memorial United Church of Christ; a veracious reader and bird watcher. She was also preceded in death by her sister. Mary is survived by her loving husband of 65 years, Howard Hensley of Miamisburg, their son Rick (Sonya) Hensley of Blue Ash, grandson Todd (Megan) and great-grandchildren Tahlia and Harrison, brother-in-law William Grothaus, many nieces and nephews.

Memorial service will be held 11:00 AM Saturday, April 28, 2012 at the Memorial United Church of Christ 2338 E. Fifth Ave. Dayton, Ohio with Pastor Ed Fox officiating. (Bob age 69 of New Carlisle, OH, went to be with his Lord and Savior on April 25, 2012 after a long courageous battle with cancer. He was born on January 16, 1943 in Dayton, OH. He was the son of the late Tolbert Horace Mattingly, Jr. and Dorthea Agnes Hayes, Mattingly.

He served his country in the United States Army. He was owner and operator of Crown Window Cleaning Service for twenty-five years and retired in 2010 after being a window cleaner for 52 years. He was a faithful member of Medway Baptist Church, where he was Head Usher. For many years he was the youth leader at his church and was loved and respected by those whose lives he touched. He loved going bass fishing with his nephew, long distance bicycle riding with family and friends, and making wood projects.

His memory will be cherished by his wife, Melody Ann Mattingly, his daughter, Sheila and son-in-law, Darren Wilson, son, Jeremiah Mattingly and daughter-in-law, Jennifer Crutchfield, Mattingly, five grandchildren, Derek and David Wilson, Stephen, Katherine, and Elizabeth Mattingly, three sisters, Patricia Roesch, Beverly Jeanne Leeth, and Kathleen Hooper, one brother, James Mattingly and numerous nieces and nephews. Hogenkamp Minster KUCH, Shirley Ann age 81, of Germantown, passed away Monday, April 23, 2012. She was preceded in death by her husband, Charles E. Kuch and parents, Glenn and Edna Harden. Shirley is survived by her brother, Richard (Marlene) Harden and family; sons, Charles G. Kuch, Doug (Sandy) Kuch, and David Kuch; grandchildren, Christina and Tamara; and other family and friends.

She was a graduate of Germantown High School. Shirley went on to receive her bachelors at Miami University and her masters at University of Dayton. She was a member of the Wesley United Methodist Church of Germantown and the Eastern Star. Shirley taught high school at View, until she retired in 1995. He was born August 11, 1941 in Dayton to the late Daniel H. and Mary E. (Knight) DeAngulo. In addition to his parents he was preceded in death by a niece, Alicia (Stalder) Allen and his dog, Maggie. Jerry is survived by his wife of 48 years, Judy A.

(Stalder) DeAngulo; his daughters, Heather L. DeAn- gulo and Diane D. Banks both of Kettering; grandchildren, Courtney (Jonathan) McFarlen of Dayton, Jerica Perkins of Riverside, Brianne Sluss of Kettering and Jerry Banks of Kettering; brothers, Daniel (Ella Mae) DeAngulo of Brookville and Hank (Karen) DeAngulo of Madison, IN; in-laws, Diane Lynch and John (Sue) Stalder; many nieces and nephews and his dog, Sadie. Jerry was a 1959 graduate of Brookville High School and completed his apprenticeship with the Plumber and Pipefitter's Local 162 in 1965. He was employed through General Motors for 20 years retiring in 1999 and from the Union in 2001.

Jerry and his wife were members and staff of the Ohio Chapter 64 of the Good Sam RV Club and were welcoming hosts at Caesar Creek State Park for many seasons. Jerry also enjoyed reading and crosswords. Family will receive friends from 11:00 am 1:00 pm Tuesday, May 1, 2012 at Gebhart- Schmidt-Parramore Funeral Home, Miamisburg. Phyllis is survived by her daughters sons-in-law, Christine Mumford Larry of Belle Center, Ohio, Michelle Fred McDaniel of Dayton, Ohio, Cheryl Les Sigmond of Greenville, Ohio and Nicole Mumford of Russells Point, Ohio; grandchildren, Alexis McDaniel, Mason McDaniel and Jenna Mumford; sisters, Eileen Marshall of Houston, Texas and Lois Beyer of Anchorage, Alaska; and a niece, Vicki White of Houston, Texas.

In addition to her parents, Phyllis is also preceded in death by her ex-husband, Robert C. Mumford Jr. Phyllis graduated from Patterson Co-Op of Dayton. Phyllis retired from Clayton Sunoco where she was the store manager. Phyllis also formerly worked at McCall's in Dayton.

Phyllis was a member of the Greenville Moose Lodge. A memorial service will be held at 5:00 P.M. on Monday, April 30, 2012 at Zechar Bailey Funeral Home in Greenville with Rev. Elwood Stuart officiating. The family will receive friends on Monday from 3:00 to 5:00 P.M.

Albert the Great Parish. Dorothy (Dot, Dottie, Memaw) lived a warm, dignified and devoted life, dedicated to her faith, family and friends. Her family was her vocation. Dorothy has been the loving mother at the center of her family that includes 6 children, 13 grandchildren and 9 great-grandchildren.
