The error occurred in information sent iO pacifier manufacturers and health-department officials that outlined required production standards and testing procedures. These were based on pacifier safety regulations that have been in place for 20 years. "They left out an item in the procedure," said Richard Viau. the acting director of Health and Welfare Canada's product-safety bureau. "It's hard to know how long it hasn't been there people have left since then but it could go back as much as 20 years." Officials discovered the error late last year and retested all models of pacifiers on the Canadian market.

Health Canada released the final results yesterday. The failure of a third of pacifier models sold in Canada may say more about the stringency of the government's requirements than it does about the products, however. Health Canada said it has not received any reports of serious injuries or deaths caused by choking on a pacifier in several years. Officials at Ste.

Justine Hospital and the Montreal Children's Hospital said they had no such cases on record. Companies whose products were found to be choking hazards have voluntarily removed their products from store shelves, Viau said. But Health Canada is warning parents to make sure they they haven't already purchased an unsafe model. Viau suggests parents throw out their children's pacifiers if they aren't certain. Any small savings from keeping an unsafe pacifier isn't worth the risk.

"My sense is: do you want to take that sort of risk for a few dollars?" he said. Viau added that parents should regularly test any pacifier to make sure it's in good shape and replace them often at least once every two to three months. Pacifiers should be replaced '7 I Ontario schools report recommends overhaul off shelves more often if the child has been taking a medication that could make saliva more acidic. Even opposition leaders said the 800-page report, For the Love of Learning, with its 167 recommendations, has to be put into action regardless of the political stripe of the next government. Liberal leader Lyn McLcod said it "will go a long way to addressing the concerns that parents have and the confusion that many people in the community have about what our schools arc expected to teach and students are expected to learn." "Frustrated parents and taxpayers want action." Tory leader Mike Harris said. "Enough study and debate let's get on ith the job of making our schools work again." w- MS If RICHARD BRENNAN WINDSOR STAR TORONTO -When Ontario's 5-pound report of the royal commission on learning, emphasizing a back-to-basics curriculum and accountability, was delivered yesterday, teachers, trustees, students and parents groups agreed it was a report whose time had come. "I hope this is the day we remember the royal commission came down and many of the recommendations were acted on in concert with all of the partners," Anne Wilson, president of the Ontario Teachers' Federation, said. "Certainly, my initial reaction is very positive," Patrick Mcany, president of the Ontario Separate School Trustees' Association, said.

Ontario Education Minister Dave Cooke said the report will have national implications. "I would suggest this report is so solid that other ministries will want to look at it and possibly a blue print for their education system," Cooke told reporters. "The accountability recommendations would put us in the forefront in accountability not only in the education system but in public services, period," he said.
