And in another incident, Canadian doctors and patients were notified of defects in about 400 artificial heart valves. "And these incidents aren't that unusual; they happen all the time," says Dr. Ajit DasGupta, head of Health and Welfare Canada's medical devices branch, who is appealing to Canadian doctors to do more to reduce the proliferation and use of deadly medical products. Doctors advised By MARGARET MUNRO Southam News The Canadian Medical Association, which has been formally asked by DasGupta's branch to report faulty medical devices everything from home tests for pregnancy to surgical lasers passed the message on to its 38,000 members.

In an article in its bimonthly journal, the association notified members of the federal request and indicated that a reporting network would be established later this year. A final decision on the service, which would be financed by the government, is to be made by the association in June. Meantime, DasGupta says problems associated with the proliferation of medical devices are mounting. More than 10,000 new devices flood onto the Canadian market each year and, unlike new drug products, the vast majority of new machines and gadgets gets no pre-market screening. "With so many new products, it would be impossible to pre-screen them all," he says, adding that the only devices that get pre-market clearance are implantable devices such as pacemakers, IUDs and drug-injection systems.

And screening of these implantable devices has only been mandatory since last year. The only requirement for the remaining 95 per cent of the 300,000 medical devices and do-it-yourself medical gadgets used in Canada is that manufacturers be able to provide data on product safety and effectiveness should the government ask for it. It's a situation that often sees health officials asking for data after someone has either been killed or seriously injured. Worse still, DasGupta and his colleagues believe most hazards and misleading claims involving the devices still go unreported and thereby unresolved. They say the big problem is getting doctors, consumers and medical workers to tell the government when problems arise.

"We can't do. anything to rectify the problems unless we know what is going on," says DasGupta. U.S. health authorities plan to tighten regulations governing the reporting of medical device failures this fall, making it mandatory for manufacturers to report all deaths and serious injuries caused by their products. DasGupta says reporting will remain voluntary in Canada, a situation he concedes will catch only a fraction of the problems.

It is hoped, however, that reporting will improve with more help from the medical profession. Of the 300 complaints the department received last year, only 60 were from doctors a surprisingly low number considering doctors have more firsthand experience with the devices than any other group of medical workers. Worldwide recall Canadian Medical Association official Martha Nosal says the problem may be that many doctors were unaware of the existence of the medical devices branch and its desire to be informed when things go wrong. The pacemaker device was tracked down this spring as part of a worldwide recall. The problem involves pacemaker "lead" device number 6972 produced by a U.S.
