8B-Spectrum Saturday, January 11, 1992 Mechanical failure not blamed for recerrt F-16 crash T.f fnl AnHv Pprnna. chief of OGDEN (AP) A Hill Air Force Base F-16 that crashed during a night mission last summer had no mechanical problems, but its night vision system was not in use at the time, Air Force investigators say. An accident investigation board report, obtained by the Standard-Examiner through a Freedom of Information Act request, said the plane's Low Altitude Navigation Targeting Infrared for Night system (LANTIRN) was in "standby mode" when the crash occurred at 10:25 p.m. near Ely, on July 30. The report questioned the lack of LANTIRN operation without concluding it was an error and made other observations of fact without assigning blame.

Killed in the crash was Capt. Keith G. Nylander, pilot and leader of a two-plane mission that left Hill nearly an hour before the crash. The mission included air-to-air refueling operations, followed by low-level navigation to the Eagle bombing range for multiple air-to-surface weapons delivery. The two F-16s had completed the refueling and Nylander was descending for the low altitude portion of the mission when the crash occurred, the report said.

According to the flight data recorder's last record, made at three seconds short of 46 minutes after takeoff from Hill, the plane was at 6,700 feet above sea level, flying at 350 mph with its nose down 11 degrees. The crash created a fireball that was observed by Nylander's wing-man, Capt. Jeffrey T. Weathers. There was no radio report from the pilot that the plane was having any problems and there was no attempt made by Nylander to eject, the report said.

Because of the remote location, the wreckage was not located until 7:30 a.m. the next day. According to the investigative report, Nylander was a "highly disciplined pilot" with 444 hours in safety for the 388th Fighter Wing at Hill, said Friday that some specific procedures involving the LANTIRN program have been im- plemented because of the crash. "We think we've avoided ever" having another crash like it and that's really the whole purpose behind these he said. Two other Hill pilots were killed Nov.
