"The door opened suddenly because it was not fully latched," said Brent Bahler, spokesman for the National Transportation Safety Board.

"The evidence is conclusive in this regard." The plane was able to return to Honolulu and land safely. The door was recovered in October, 14,100 feet below the ocean surface. Bahler called the door's retrieval an investigator's dream because it allowed hands-on examination.

In April, the NTSB issued a probable i cause findingconcluding that the lock sectors caused the accident by allowing the door to appear closed when it was not They recommended strengthening the sectors. Investigators do not anticipate making any new safety or design recommendations, Bahler said. "They feel the previously adopted recommendations are sufficient to prevent this kind of accident from happening again," he said. But investigators are puzzled because certain mechanisms in the door's locking system were open, while others were locked, Bahler said. Boeing had designed the lock system to be error-free, using hooking pins, latch cams and lock sectors to bind it to the plane's fuselage, Bahler said.

The door first is pulled shut by the hooking pins. Eight C-shaped latch cams then rotate around steel latch pins mounted on the fuselage to lock the door against the aircraft The L-shaped lock sectors then swing over the latch cams to prevent them from rotating open. "The door latch cams were found very close to where we expected to find them: not in the fully latched position. But lock sectors found in the locked position, driven past the fully locked position were not in the condition that investigators had suspected they would be," Bahler said. i The cause of the "back driving" has not been determined although there can only be two reasons: mechanical or electrical failure, Bahler said.

Damage to the latch cams may have occurred when the door hit the water or from lengthy submersion, he said. 1 NTSB officials do not know when their final report will be completed or if human error will be blamed. 1 Investigators are "very close" to com- i pleting mechanical, metallurgical and electrical examinations of the door, i Bahler said. The medical examiner's office identified the victim as Harold Rawlins of Ewa Beach. His death was classified as the 89th traffic fatality of 1990.

Rawlins was walking east on Pohakupuna Road at 7:25 p.m. on Dec. 23 when he was hit by a car going in the same direction. Freeman, special agent in charge of the Honolulu FBI office. The bandit entered the bank at 1283 Kapiolani at about a.m, handed a note to a teller demanding money and threatened to use a weapon. Todd Mueller, 23, was arrested last week in Chicago with another former Oahu Community Correctional Center inmate, 27-year-old Roy Hartsock, for trying to cash stolen checks. Honolulu police had been looking for Mueller on forgery warrants. Forging "Bank of Iowa" checks last year are among the scams Mueller is suspected of pulling off in Hawaii There is no Bank of Iowa and even though the location of the branch was listed as Boise which is in Idaho some fake checks were cashed.

In June 1988, Mueller tried to hang himself at the Halawa Medium Security unit. He had been convicted of first-degree theft in August 1986. Hartsock, meanwhile, is wanted locally for prison escape. In prison for robbery and drug convictions, he was released from Oahu Community Correctional Center last month on orders signed by Circuit Court Judge Marie Milks. It turns out that Milks' signature was forged.

Hartsock was cleared for extradition today and will return to Hawaii to face charges, possibly by this weekend. Mueller is still being held in Chicago.
