Tribune 5 Agents fear reforms will cost them their jobs long, drawn-out battle' promised By CHRISTOPHER CONNELL Associated Press WASHINGTON (AP) Insurance agents are up in arms over health reform, fearful that the White House is hatching plans that could put thousands of them out of work. They are hiring powerful lobbying firms and raising money to do battle if necessary against President Clinton's resolve to reshape the way most Americans get their health insurance. Joseph Lee Pugh, president of the National Association of Health Underwriters, put it bluntly in a recent appeal to his colleagues: "Are you prepared to lose your Unless they contributed $200 apiece to an emergency, $1.2 million war chest, he warned, agents "could go the way of the in the next year." Experts and some White House advisers agree that health reform could wipe out thousands of jobs in the insurance industry, from the agents who specialize in selling policies, to small businesses, to the insurers' home office staffs and medical underwriters. Just last week, Hillary Rodham Clinton, who is chairing the White House health care task force, predicted that reform will trigger "a Darwinian struggle" for survival within the industry. The president wants to make it illegal for companies to deny insurance to people with medical problems and wants to force companies to sell policies at the same rates to everybody within a community. Under Clinton's vision for managed competition, small businesses and individuals would buy insurance through huge, government-created cooperatives in each state selling a limited number of plans. Some of the biggest names in the business, including Prudential, Aetna and Metropolitan Life, are already major players in the managed care market and are well positioned to profit from Clinton's push in that direction.

But the future is much less secure for smaller companies and the independent insurance brokers. The brokers' cut typically runs from 5 percent to 10 percent of the premium. It adds to overhead that can claim 30 percent or more of the premiums paid by small businesses. Lindsay said he may seem like an unnecessary middle man to the health reformers in Washington, but to his cliencs, "I'm their human resources department." "They hire me to make their life easier so they can go about their business. The dry cleaner wants to take in clothes and clean them and get them out.

He doesn't want to learn federal rules and regulations," he said. David Saltzman, a broker in Miami, said, "Have you ever called a government agency and tried to get a clear answer to a question in less than halfaday'stime?" "We help explain choices. Today we have HMOs, PPOs, point of service plans," said Saltzman. "I have not yet been to a client, no matter how sophisticated, who understands the differences in all of those health plans." Even if Clinton creates insurance purchasing cooperatives, he said, consumers will still need agents to tell them "the differences between Plans A.B.C.DandE." "If insurance companies could find a way to sell their products less expensively without using agents, they'd do it tomorrow," said Alan Katz of Woodland Hills, Calif. He is with West Coast Multiple Services, a big brokerage.

Until recently, Neil Trautwein was the only lobbyist for the member National Association of Health Underwriters. It recently hired the lobbying firm of Patten, Boggs and Blow. The larger National Association of Life Underwriters, which has nine in-house lobbyists, also has turned to Patten, Boggs and Blow and enlisted a second lobbying firm, Van Scoyoc Associates. "This is going to be a very long, drawn-out battle," said David E. Hebert, government affairs counsel at the NALU, which counts 143,000 members in its state and local affiliates and has 8,000 brokers in a subsidiary, the Association of Health Insurance Agents.

"We're hoping that Capitol Hill will be a different ball game." Bill Gradison, president of the Health Insurance Association of America, said the agents make "very, very effective" lobbyists for themselves. "These are the pillars of their communities. These are the folks that coach the Little League and belong to the Rotary Club and sit across the kitchen table to help people develop a financial plan for their families," said Gradison, a former Republican congressman from Ohio.
