To be eligible for enrollment, a child must have reached the age of 6 years on or before next Oct. 15, Fritzley said. 1 Parents are requested(o bring the child's birth certificate or other proof of age, dates and types of Immunlutlons received and dates of any serious Illnesses or Injuries. Parents preferring to take their children to their family doctors for the pre-school checkup should still register them at school on May 5 and present a doctor's statement of examination, Fritzley said. Children who will be examined at the clinic are asked to wear easily removed cloth- Ing and to bring a robe or large towel. DPT and small pox boosters will be given to those who want them. A donation of 50 cents per shot to cover cost of sup. plies would be appreciated from those able to contribute, Fritzley said.
