The forms also are available in the emergency rooms of Port Huron and Mercy hospitals, Yale Community Hospital and River District Hospital. The forms may be a shortcut to quicker treatment for the ill child. Otherwise, "unless the case is an extreme emergency, a child must wait until his parents can be reached for their consent before he can be treated," Fletcher said.

Carrigan said the problems associated with out-of-town parents Is worse in the McCill educator to give last Lenten lecture Dr. William Lawlor, of Montreal, will give the last lecture of the Newman Center Lenten Series. Port Huron Hospital and the Noon Optimist Club of Port Huron have begun a project designed to answer that question. Called "For Your Child's Sake!" it provides parents with forms that authorize medical treatment when needed.

The forms, when signed by parents and witnessed, authorize the hospital to perform medical and surgical treatment. When completed, the forms Include telephone numbers of police, nearby hospitals, physicians, relatives and neighbors. The forms also include the parents' destination, the type of car they are driving and their principal travel route. The child's allergies and medication and the date of his last tetanus shot also are listed. Donald Fletcher, associate executive director of the hospital and a member of the Optimist club, obtained the backing of the club when the hospital proposed the project.

Margaret Carrigan, Port Huron Hospital executive secretary, designed the Fire destroys vacant home, damages second ALGONAC A fire early this morning destroyed one home and partially destroyed another in the city's 118 block, an area of condemned homes in the urban renewal area. Police Chief Robert Christy said a mo-' torist reported the fire at Michigan and Summer streets shortly after 3 a.m. The police then called the Algonac Fire Department, who put out the blaze by 4 a.m. The fire department said it will conduct an arson investigation. Thursday. Woodrow G. Brogan NEW HAVEN Woodrow G. Brogan, 59, of 59727 Havenrldge Road, died Tuesday in St. Joseph Hospital West, Clinton Township, after a long Illness.

He was born Jan. 10, 1918, in Ruth, W.Va., and lived In New Haven 23 years. He married Delorls Dureckl Sept. 7, 1946, in Greenup, Ky, Mr. Brogan was a veteran of World War II, serving with the U.S.

Navy. He was a retired employe of Kehrig Manufacturing Mount Clemens. He was a member of Chapter 44, Disabled American Veterans. He is survived by his wife; nine sons, John Brogan, Virginia Beach, Jerry Brogan, Elkhart, W.Va.; Danny Brogan, St. Albans, W.Va.; Lawrence Brogan, Detroit; David, Patrick and Christopher Brogan, all of New Haven, and Richard and Thomas Brogan, both of Armada; four daughters, Mrs.

Frances Goerlich, Mount Clemens; Carol A. Brogan, Detroit, and Jane and Donna Brogan, both of New Haven; a sister, Mrs. Epsie Kincald, Roanoke, a brother, Ray Brogan, St. Albans, and 11 grandchildren. Services will be at 1:15 p.m.

Hicks NAPLES, Fla. Leroy E. Hicks, 82, former Huron County resident, died Sunday in Naples. He was born Dec. 12, 1894, In Redman In Huron County.

A former resident of Bad Axe and Port Austin, he had lived in Naples since 1945. He married Miss Clara Broomfleld In September, 1919. She died June 2, 1934. He married Miss Vivian Fager in 1938. Mr.

Hicks was a U.S. Army veteran of World War I. He owned and operated Hicks Restaurant, Bad Axe, from 1920 to 1944, when he retired. He had owned a restaurant In Port Austin for many years. Mr.

Hicks was fire chief In the Bad Axe Fire Department. He was a charter member of Sherman Elbin Post 96, American Legion, and the Lions Club. He was a life member of Verona Lodge 365, AM. He was a drum major in the drum and bugle corp from 1932 to 1950. He Is survived by his wife; three daughters, Mrs.

Willett (Betty) Rondeau, Westgate, Mrs. Charles (Genevieve) Todd, Bad Axe, and Mrs. Clayton (Jean) Harrold, Naples; two sons, William M. Hicks, Oahu, Hawaii, and James D. Hicks, Las Vegas, a sister, Mrs.

She married Howard Munro Sept. 15, 1929, in Detroit. Mrs. Munro was a gradu-. ate of Almont High School and Ypsilanti state Normal College.

She taught school in St. Clair and Sanilac counties from 1926 to 1967. She was an active member of Peck United Methodist Church, leading the children's choir and teaching Sunday School. She was a member of the Methodist Women's Society. Mrs.

Munro is survived by her husband; a daughter, Mrs. Robert (Eleanor) Grout, Croswell; three sisters. Mrs. Hazel Allen, Yale, Mrs. Mary Napolitan, Detroit, and Mrs.

Florence Bubel, Rus-kin, a brother, Percy Edgerton, Riley Center; and six grandchildren. A son, James Munro, died Sept. 28, 1969. Mr. Davidson was born Nov. 7, 1899, in Old Springs, and moved to Battle Creek in 1941. He married Ivy May Thompson Aug. 25, 1926, in Port Huron.

She died Feb. 12, 1976. Mr. Davidson worked with the Veter ans Administration Hospital, retiring in 1963. He and his wife operated Davidson Day Care Center until 1970.

Mr. Davidson was a member of First Baptist Church, Ira A. Berk Lodge 503, AM, and the American Association of Retired Persons. He is survived by a daughter, Mrs. Leonard (Marlene) Asonoff, Battle Creek; two sons, Richard A.
