Drug offers hope to schizophrenics CHICAGO (AP) A new drug that has been shown to help onethird of previously untreatable schizophrenics with irreversible brain damage may offer insight into the causes of the disease, researchers say. Although the drug has a potentially fatal side effect, it is the first drug to show improvements in patients whose brain damage suggested they were beyond help of drugs, said Dr. Herbert Meltzer of Case Western Reserve University in Cleveland, one of the directors of a new study of the drug's effectiveness.

"I would predict within three to five years we'll have a safer version of this drug and we'll know more about schizophrenia." Meltzer said Thursday at the annual meeting of the American Psychiatric Association. He and his colleague, Dr. John Kane of Long Island Jewish Hospital in New York, reported Thursday that about 30 percent of the estimated 300,000 Americans with untreatable schizophrenia improve with the drug, called clozapine. "It's a major, major advance," Meltzer said. Many other so-called neuroleptic or anti-psychotic drugs are used to treat schizophrenia, and they are all about equally effective, Meltzer said.

but are of no value in the most severe cases of schizophrenia. "This is the first time in the history of these drugs that any one antipsychotic drug has been shown to be superior to any of the others." Meltzer said. Dr. Fritz Henn of the State University of New York in Stony Brook said the drug "tells us that it is possible to make significantly better anti-psychotics," and so is likely to new ADJUSTABLE RATE CD Now Available At The Marion Center National Bank TERM: 18 Months INTEREST RATE: 50 Basis Points Above The 90 Day U.S. Treasury Bill Rate (the current rate is 6.23%) INTEREST ADJUSTMENT: The interest will be adjusted the last calendar day of each quarter.

He said, however, he is less enthusiastic about the drug's effectiveness and concerned about its potentially fatal side effect, a loss of white blood cells, crucial in fighting infeclions. Meltzer said no deaths have occurred from the drug in the United States and that the side effect. called agranulocytosis, is reversible if it is detected early and drug treatment is stopped.

Eight Finnish patients died while receiving the drug around 1975, Meltzer said, a development that halted for several years many studies of the drug's effectiveness. Meltzer has found that the blood disorder can be detected and reversed in time to prevent death if weekly blood counts are taken of patients receiving clozapine. Henn said such careful observation might not always be possible. "Many of our patients are in state institutions where the levels of care are variable," he said. Other anti-psychotic drugs have about a one in 1,000 or one in 1.500 chance of producing this effect.

Meltzer said. With clozapine, the chances of this occurring are about one in 100, he said. The drug was tested on approximately 125 severe schizophrenics in Meltzer's study. A similar group of schizophrenics received chlorpromazine, a widely used anti-psychotic drug. After six weeks, 30 percent of those receiving clozapine showed "significant clinical improvement," Meltzer said.

Of the patients given chlorpromazine, 4 percent showed an improvement, he said. Clozapine may be superior to other anti-psychotics in terms of another side effect called tardive dyskinesia. Goetz is charged with the Dec. 22, 1984, subway train shooting of four young men he claims were about to rob him. Goetz at the time was hailed as "the subway vigilante" and sympathizers established a Bernhard Goetz Legal Defense Fund that collected thousands of dollars within weeks. Goetz declined to use the money to pay his bail, and Slotnick said the public incorrectly assumed he was spurning all contributions. The defense fund is still accepting donations, and that money is being used.

"There are certain experts that we would have wanted to bring in, but unfortunately their fees have been above and beyond what we have been able to acquire with the defense fund," Slotnick said. This condition occurs in about 20 percent of patients receiving conventional anti-psychotics, and their chances of getting it increase the longer they stay on medication. Meltzer said.
