Blockage causes middle ear buildup Dear Donohu*: Last year I experienced some fluid buildup in my middle ear. This was accompanied by nausea and dizziness. I took Antivert. It took five weeks to clear up. Here it is a year later and I'm experiencing the same conditions. I am back on An- tivert, and also am taking Erythromycin. I still don't feel any better. Can you please elaborate on the middle ear buildup? My equilibrium is off also. — Mrs. M.R. The middle ear is a small, hollow cavern in the skull between the eardrum and the inner ear. It contains three tiny, linked bones that vibrate to amplify sound for us. This bony cavern has to be kept drained so that Just the right amount of pressure exists within. Normal drainage is accomplished via a tiny tube to the back of the throat (eufltachian tube). This is where fluid accumulation problems often arise. If the tube is blocked, as it can be from a throat infection, for example, the vital connection to the middle ear is blocked, pressure there is lowered, and the vacuum effect forces tiny blood vessels to leak fluid. That is called serous otitia media — fluid buildup. There are other contributing causes. Allergies can lead to a blockage of the tube, as can infection within the middle ear itself. Whatever the cause, fluid buildup should not be allowed to exist for long periods. Hearing will be affected. Nausea and dizziness occur because balance and the other mechanisms are so intimately related within the ear structure. The goal is to drain off the extra fluid, and this can be done through use of antihistamines (your Antivert-mecl- izine) and decongestants. If that does not work, the fluid can be drained mechanically through a small tube placed in the ear drum. The Erythromycin is an antibiotic, which helps fight off any infection present. * -tr -tr Dear Dr. Donohue: I am a 26-year- old female and have had recurrent bursitis in my shoulder for the past eight years. No calcium deposits have been found. I have been getting the attacks every few months whether or not I use the shoulder much. Would you have any idea why I get this bursitis so often. Should I Just ease the pain as it occurs and live with it? — Mrs. R.L.S. My first instinct is to question whether this is bursitis at all. You may have that, but it would be wise to get a professional opinion on the matter. Tendo- nitis of the large cuff of muscle ten- dona at the shoulder Joint (rotator cuff) may be the problem. Bursitis, as you may know, involves a different structure — the tiny sacs that help tendons slide easily over Joints. Inflammation of another tendon, the bicipital tendon, can cause shoulder pain. What I am trying to suggest is that there are many conditions that cause shoulder pain, bursitis being but one. Perhaps there are some simple exercises that you could do to stave off attacks. Or perhaps attacks could be aborted by prompt use of anti-in- flammtion drugs, rest and ice at the first sign of trouble. That's not much of an answer, but I want to emphasize that you need diagnosis before you can hope to end your problem permanently. •fr -fr * Dear Dr. Donohue: How did St. Vitus Dance get its name? With this, muscles are weak and there are involuntary, purposeless movements. The arm may reach out; the leg may Jump. The movements disappear with sleep. It was at one time a fairly common result of rheumatic fever, an illness that has been largely eliminated because of the use of penicillin. The penicillin combats strep throat infection promptly to prevent both rheumatic fever and the nerve-related symptoms. I don't know the history of the name St. Vitus Dance. I have never seen a church with that name, only this illness. It is not contagious.
