He was 68 years old.

Mr. Levenson, most noted for his books chronicling his family life, died in the coronary care unit of Long Island College Hospital. He had been brought to tne hospital earlier in the day. The youngest of eight children of Hyman and Rebecca Fishelman, Mr. Levenson was born Dec.

28, 191 1, on the Lower East Side of Manhattan then a crowded corridor of European Jewish immigrants. He graduated from Brooklyn College in 1934 and taught high school for 12 years. After his first appearance as a comedian, before a teachers convention, he entered show business in 1946. He was a popular performer on the banquet circuit, and he also appeared as a panelist on several television shows, including "This Is Show Business," "The Match Game" and "To Tell the Truth." In addition, he was host of "The Sam Levenson Show" in the early '50s, moderated "Two for the Money" and appeared as a guest host on "The Ed Sullivan Show." In his later years, Mr. Levenson's humor had a serious edge. "As time goes on," he said, "I find I get more satisfaction out of saying something important than saying something funny." Surviving are wife, Esther, and their two children, Conrad and Emily.

St. Louisan Jimmy Forrest Dies; Was Jazz Musician Funeral services for St. Louis-born composer and jazz musician Jimmy Forrest will be held tomorrow in Grand Rapids, Mich. Mr. Forrest, who played tenor saxophone, died of a heart ailment Tuesday night in Grand Rapids.

He was 60 years old. The Sumner High School graduate composed the hit instrumental "Night Train" and recorded the popular "Blue Groove." He began playing professionally in St. Louis at the age of 13 and later played with such musical greats as Duke Ellington and Count Basie. He and another Basie band member, trombonist Al Gray, started their own band three years ago. In the book "Jazz Masters of the Forties," author Ira Gitler wrote: "Jimmy Forrest, who played with (Jay) McShann during (Charlie) Parker's time with the band and later worked with Duke Ellington, was playing material like "Little Willie Leaps" and "Our Delight" when I heard him in St.

Louis with his own small group in 1947, and his solos showed a complete grasp of the new idiom In the 60s, with Harry Edison's combo, Forrest played a vigorous mainstream-modern vein." In his early years here, Mr. Forrest was the mentor of jazz great Grant Green, who gained international fame as a guitarist; Green died last year of a heart ailment in Detroit. Mr. Forrest often returned to St. Louis as a musician, twice in 1976.

Of his appearance here in March of that year, a reviewer wrote, "Forrest took a lot of solos and was a big hit around intermission time as was Basie, who was swamped by autograph seekers." Mr. Forrest and his wife of almost three decades, Almira, were divorced last October. He married for the third time and was living in Grand Rapids with his wife and a daughter from his first marriage, Candy, when he died.
