If there were an actual, attack, doctors' failure to isolate contagious patients with smallpox or plague could increase the number of victims. "The risk of spread goes up logarithmically," said study co author Dr. Stephen Sisson of Johns Hopkins University School of Medicine.

The lesson of the 2001 anthrax attacks, when at least four patients first were sent home without a diagnosis, has not yetbeen learned, Sisson said. States and medical residency programs 'We've got a dangerous gap here and we need a much clearer strategic game Shelley Hearne executive director Trust lor America 8 Health 'i i i i i i should require bioterrorism training for doctors, he said. The study fn Monday's Archives of Internal Medicine involved 631 doctors, mostly medical residents doctors still in training in 30'internal medicine residency programs in 16 states and Washington, D.C. The doctors took a pretest, completed an online course and then took another test. On the pretest, half the doctors misdiagnosed botulism; 84 percent archinternmed.com misdiagnosed plague.

A case of routine chickenpox was misdiagnosed by 42 percent of tho doctors as smallpox "We've gota dangerous gap here and we need a much clearer strate 1 gic game plan," said Shelley Hearne, executive director of Trust for America's Health, which tracks how well states are prepared for bioterrorism and pandemics. While it's encouraging that the Department of Homeland Security recently created the post of chief medical officer, Hearne said, funding for training is an ongoing problem, 1 In fiscal year 2005, Congress approved $252 million for health professions training activities" in the Health Resources and Services Administration, a line item eliminated in President Bush's proposed 2006 budget, she said. The study, she noted, shows training Works. After the online training, the doctors' correct diagnoses averaged almost 80 percent, Before the training, correct answers averaged 47 percent. Dr.

Rika Maeshiro of the Association of American Medical Colleges said that from 2000 to 2005, the proportion of medical schools requiring biological and chemical terrorism training jumped from 16 percent to 83 percent which could improve results if the researchers repeat the study. Last month, a similar study by the Rand Corp. found that some public health clinics gave bad medi ical advice by phone to researchers who posed as doctors describing symptoms of botulism, plague and smallpox. The Rand report said some callers who described of bioterror relat ed illness were told not to worry. Reputed mob boss dies in prisoivat 77 CHICAGO Albert "Caesar" Tocco, a reputed mob boss.who was sentenced to 200 years after his wife took the unusual step of testifying against him, died Wednesday in an Indiana prison.

He was 7f. Tocco died in a federal penitentiary in Terre Haute, alter suffering a stroke, said Mike Truman, spokesman for the Federal Bureau of Prisons. He was 15 years into his prison sentence for racketeering, conspiracy, extortion and tax "He allegedly oversaw organized crime operations in many of Chicago's southern suburbs. Tocco was arrested in Greece in 1989 and was convicted in federal court In an interview published in thq Chicago Sun Times in 1990, his wife called her husband a ruthless thug who abused hist family and even cheated his daughter at tic tac toe. The Associated Press Unexplained weight loss may be sign of Alzheimer's The Associate! Pro CHICAGO Unexplained weight loss in older people might be an early signal of Alzheimer's disease, appearing several years before the memory lapses that define the illness, according to an intriguing but unproven new theory.

Researchers at Chicago's Rush University Medical Center base the theory on their study of 820 Roman Catholic priests, nuns and brothers aged 75 on average who were followed for up to 10 years. Otherwise healthy participants whose body mass index fell the most were the most likely to develop Alzheimer's disease. Study co author Dr. David Bennett, director of the Rush Alzheimer's Disease Center, says tne results raise the possibility that the disease attacks brain regions involved in Regulating food intake and metabolism, as well as memory, and that weight loss is an early symptom. Weight loss frequently occurs after an Alzheimer's diagnosis and has been attributed partly to memory lapses or lifestyle 'The idea that something would start before it became clinically obvious no longer Seems that Dr.

Peter Rabins Alzheimer researcher changes associated with becoming infirm. But it might be that brain changes that start well before diagnosis are another reason, Bennett said. The results appear in today's edition of the journal Neurolo gy Dr. Peter Rabins, an Alzheimer's researcher and professor of medicine at Johns Hopkins School of Medicine, said the research fits with an increasingly popular belief that Alzheimer's abnormalities "really are present for at least 10 years before there are any symptoms" "The idea that something would start before it became clinically obvious no longer seems that far fetched," Rabins said. However, Rabins said he thinks it's likely that gradual weight loss stems from subtle behavior changes such as loss of initiative, which could result in less snacking or eating out, than in brain changes affecting metabolism.

Those behavior changes, involving parts of the brain associated with Alzheimer's, often precede the diagnosis, but "because it's a subtle thing, it's often not recognized except in retrospect," Rabins said. Dallas Anderson of the dementias of aging branch at the National Institute on Aging, which funded the research, said the results are intriguing but that the theory needs further testing in a more diverse group. When the study began, participants' average BMI was 27.4, in the overweight zone, and none had, been diagnosed with Alzheimer's. By study's end, 151 Neurology; www.neurolo gy org National Institute on Aging: www.nio.nih.gov had been diagnosed with the disease, Those whose BMI dropped a point each year faced a 35 percent increased risk of being diag nosed with Alzheimer's later on, compared with those whose. BMI remained stable.

Dr. Deborah Blacker, director of gerontology research at Massachusetts (general Hospital in Boston, said depression some times precedes Alzheimer's and can also cause weight loss, which might at least partly explain the People tend to lose weight In old age because of loss of bone and muscle mass, but the researchers said factoring in age, chronic! disease, gender and other characteristics that might have affected weight didn't change the results.
